Warning letter format needed for absenteeism?

Hi, I am looking warning latter format for absenteeism employees. We need to send warning who are absent and abscond after they getting the salaries. Please help.

    Subject:- Warning Letter for absenteeism.

    Dear (Name of Employee)

    It has been observed by our authority members that after your salary is credited to your account you remain absent for a few days without taking any prior permission from our concerned authorities. This shows your irresponsible behavior and you are open to disciplinary actions. 

    We value attendance and it is very important for your position. This type of irresponsibility is not acceptable otherwise we have to take strict action against you. 

    Remember to always submit your written application three days prior to taking any leave. And there must be a genuine reason for your absence otherwise we have to deduct your monthly salary. 

    We will also put a copy of this letter in your office’s personal file. Kindly, take this warning seriously.
