Dear Sir,

My name is Annie Samantha, Employee Id 12345, and I am working as Senior Executive in the Accounts Department of  Loanbazaar Inc.

I would like to inform you that I have been blessed with a baby boy. I was granted a maternity leave of four months from 17^th Feb, 2022 to 20^th July, 2022.

I am writing this letter to request for extension of leave as I have not been able to completely recover from my pregnancy. Therefore, I would appreciate to extend my maternity leave until  15th August, 2022.

I will get back to work from 16th August, 2022, right after the extended leave date. I hope you will consider my situation and approve my request. I humbly apologize for the inconvenience caused because of my leave extension.
